Chris Rose-Franks

Chris joined the CSC team in 2020 with a plethora of professional experience under her belt. As a Certified Care Manager, Chris is uniquely qualified to manage all aspects of our client’s healthcare needs with proficiency and respect. Chris served in both clinical and managerial positions the past 20+ years as a Geriatric Care Manager for a home care agency, a Care Coordinator in Case Management for a hospital, as an Executive Director at assisted living facilities, and as a Physical Therapist Assistant in a variety of rehab settings.
Chris served for many years on the Board of An Elderly Wish Foundation, a non-profit group that grants wishes to elderly residents of Contra Costa County with life threatening illnesses. She also has a passion for helping families navigate through their loved one’s journey with dementia with its myriad of challenges.
In her free time, Chris enjoys spending time with her husband and son and is an avid baseball fan of her son’s college team as well as of the Oakland A’s. She also enjoys photography, long walks with her Labrador, sharing laughs with friends and family, and time spent in nature.
Chris earned a Bachelor of Science degree in Kinesiology & Physical Education in 1987, followed by a Master’s in Public Health Administration (MPA) in 1997. She is a licensed Physical Therapist Assistant (PTA), a Certified Care Manager (CMC), and an Advanced Professional member of the Aging Life Care Association (ALCA).
